Scheduling an appointment

If you'd like to schedule an appt. with us, simply call and we'll begin the process. Our in-take coordinator will try to get 95% of your information over the phone.

You will receive a new patient information packet that you will need to fill out and bring in with you to your appt or you can print them from our website and bring with you. You will also be advised to what other information you should bring with you to your appt. Usually, it's a prescription from your physician, any past medical records pertaining to the reason you are seeing us, and your medical insurance cards.

Insurance and payment

We will call on your insurance benefits and do any preauthorization steps for you prior to the delivery of an item. You will be responsible for any unmet deductible and co-insurance at the initial appointment. At the time of delivery, your insurance will be billed.

We are contracted with the following insurance companies: Blue Cross, Idaho Physician's Network, V.A., PacificSource, and Tricare. We submit claims to all other insurance companies including Medicare and Medicaid as a courtesy to our patients. However, we do NOT bill Healthsense 65 or process auto insurance claims.
