Scheduling an Appointment

If you would like to schedule an appointment with us, please call 208.342.4659 or submit your request HERE. For most all of our services, a signed prescription from a physician is required.

At the time of your appointment, you will be asked to fill out a patient registration form, or if you would prefer to expedite your visit, you can download and complete our patient registration forms HERE. We will also require a copy of your medical insurance card(s), photo identification, and a prescription from your physician.

Insurance and Payment

We will call on your insurance benefits and do any necessary pre-authorizations. You will be notified after the initial evaluation appointment by a member of our billing team, to inform you if there is any unmet deductible and/or co-insurance payment for which you would be responsible. If there is an estimated amount due by you, we would collect that portion at delivery. We cannot bill insurance until you have received your item.

We are in-network with most insurance companies. We can submit claims to all other insurance companies as a courtesy to our patients, however, we do NOT process auto insurance claims.
